The adoption numbers for AI agents in marketing are striking. Ninety point three percent of marketing organisations report using AI agents in some capacity. That figure suggests near-universal deployment and might lead to the conclusion that agent adoption is a solved problem. The Gartner forecast for 2027 complicates that picture significantly: 40% of agentic AI projects will be cancelled before they reach mature deployment, and the cause is not the technology.

Fifty-three percent of marketing organisations have no AI governance framework in place (MarTech Weekly, March 2026). Deploy agents into an organisation with no governance, and you get the pattern Gartner is forecasting: early enthusiasm, scope creep, quality problems that erode trust, and eventual cancellation of a project that was technically functional but organisationally unmanageable.

The 60% that succeed do specific things before they deploy. The 40% that fail skip them.

Why Agents Fail Differently From Other AI Tools

Standard AI tools (copilots, chat interfaces, content assistants) operate with a human reviewing every output before it has any external effect. The human is the checkpoint. If the output is wrong, the human catches it before it causes a problem.

Agents operate with more autonomy. They execute multi-step workflows, make intermediate decisions, call external APIs, and take actions (sending emails, updating records, posting content) with limited human oversight in the middle of the workflow. This changes the error profile dramatically. A mistake by an autonomous agent can propagate through multiple downstream steps before a human sees the output. By that point, the error has effects that cannot simply be undone.

This is why governance matters more for agents than for assistive AI tools. The cost of an unreviewed bad output is higher, the error can travel further, and the complexity of the system makes it harder to identify what went wrong after the fact.

Three Pre-Deployment Questions That Separate the 60% from the 40%

Question 1: What does this agent do when it is uncertain?

Every agent will encounter situations its designers did not anticipate. The question is what happens in those situations. Does the agent attempt to complete the task using its best guess? Does it flag the uncertainty and request human input? Does it halt and log the situation for review?

Most failed agent projects did not define this behaviour before deployment. The agent was designed for the expected cases, and the unexpected cases were not systematically considered. When the unexpected cases arrived, the agent behaved in ways that were technically within its design but problematic in their effects. A content publishing agent that cannot determine whether a post is ready might publish something incomplete. A customer communication agent that cannot classify an unusual query might route it incorrectly. These are not technology failures. They are design omissions.

Before deployment, define explicitly: at what level of uncertainty does the agent stop and request human review, and who receives that review request and with what information. This single design decision prevents a significant fraction of agent-related quality failures.

Question 2: What is the acceptable error rate, and what happens when errors occur?

Every agent will make some errors. The question is whether the error rate is acceptable for the task, and what the response process is when errors are identified.

Acceptable error rate depends on the cost of an error in context. A research synthesis agent that is wrong 5% of the time is acceptable if a human reviews the synthesis before acting on it. An automated customer email agent that is wrong 5% of the time may not be acceptable if wrong emails go to customers at scale. The same error rate has completely different acceptability depending on the stakes of the task.

Defining acceptable error rate before deployment forces two important conversations: whether the current agent design can achieve that error rate (if not, the design needs to change before launch), and what the review and correction process is when errors occur. Teams that do not define this before deployment often discover the error rate after deployment, when errors have already had effects, and then add reactive review steps that make the workflow slower than it was before the agent.

Question 3: Who is accountable for agent outputs, and how does that person know when something is wrong?

AI agent projects that succeed have a named human accountable for the quality of agent outputs, with a defined mechanism for how that person receives information about agent performance. Projects that fail often have diffuse accountability (everyone assumes someone is watching) and no systematic reporting on agent performance.

Accountability without monitoring is accountability in name only. The accountable person needs a dashboard, a log, or an alert system that surfaces anomalies, errors, and performance trends without requiring them to manually review every output. This is not technically complex. Most agent frameworks include logging by default. The design decision is what gets logged, how it gets surfaced, and who acts on it.

Building for Reversibility

A cross-cutting principle for agent deployment that separates robust from fragile projects: design for reversibility wherever possible. Prefer agents that draft rather than publish, that queue rather than send, that flag rather than act, at least in early deployment phases. The reversibility constraint makes errors recoverable rather than permanent.

As trust in the agent builds (demonstrated by consistent performance over time, documented error rates within acceptable bounds, and a track record of handling edge cases correctly), autonomy can be progressively increased. This is the progression from draft-and-review to fully autonomous that experienced AI practitioners follow. It is also the progression that the 60% of successful agent projects follow, and that the 40% who skip straight to full autonomy do not.
